RMA Form Instructions

Use the RMA form to request a Return Merchandise Authorization (RMA) number before returning material to SimPhonics. When returning equipment to SimPhonics, Inc. please observe the following instructions:

  1. Request an RMA Number

  2. You must obtain an RMA number before returning material to SimPhonics. This is done by completing the online RMA form.

Before submitting an RMA form you need:

  • Serial number of the computer system in question. For fans, memory or misc computer components we only need the serial number of the COMPUTER SYSTEM the component came from.

  • Part number of computer system in question. For fans, memory or misc computer components we only need the part number of the COMPUTER SYSTEM the component came from.

  • Name of a contact at your company

  • Detailed description of the problem with the equipment

  • Date of delivery

SimPhonics will notify you of your RMA # by E-Mail or Phone. 

All RMA #'s are valid for 14 days from the issue date.

  1. Review Your Warranty

    Please note the length of the warranty of the returning item(s).  If out of warranty, you need to send a check or Purchase Order for our bench fee of $225.00 per item. See more information regarding warranties.  

    Also note that many Original Equipment Manufacturers (OEM) warrant their products for many years. If your SimPhonics' warranty has expired, please check with the OEM for warranty and repairs.

  2. Packaging Your Equipment

    After receiving your RMA number, use the following steps to prepare your package for shipment:

  • Use a box large enough to allow for adequate cushioning material on all sides of the equipment.

  • To properly cushion the contents of your package, wrap each item separately. Use proper electrostatic discharge (ESD) precautions.

  • Include in your shipment a description of the problem and a point of contact at your company.

  • Do not include any accessory pieces such as mounting brackets, power cords or software.

  • Firmly tape the lid of the box with packaging tape.

  1. Labeling Your Package

    When shipping a package, include your RMA number on the label. Send the package to the following address:

Note:  

If out of warranty, you need to send a check or Purchase Order for our bench fee of $225.00 per item. 

All RMA #'s are valid for 14 days from the issue date.

Failure to follow these instructions may result in your shipment being refused or your RMA not being processed.

Items can only be returned for restocking within 90 days from the date of shipment.

There is a limited warranty on "repaired" items of 90 days from the Date of Repair (DOR). DOR is the date on the Test Report or Shipment documents from SimPhonics or SimPhonics' vendor.
